New Client Intake Forms
Congratulations on taking this step to enroll in my VIP Parent Coaching program! I am honored you have selected me to be your coach and am excited to work with you. More importantly, I’m excited to see who you become through this process. The journey isn’t always easy, but it is definitely worth it! Before we jump into all the good stuff that is waiting for you, we must attend to the paperwork.
Step #1: Welcome Letter
- This document offers some brief instructions for getting started, accessing the Parent Transformation Academy, scheduling your sessions. and getting started with Zoom. You will have a unique session login assigned to you to use for all our sessions. I will send this to you once I have all your completed paperwork.
Step #2: Coaching Policies and Procedures
- Please download and review this document by clicking the button above. This document contains everything you need to know about coaching, being coached, and a whole lot more…including session attendance, office policies, confidentiality, and financial agreements. You will be asked to verify that you have read and understood this document in the Coaching Service Agreement. You are advised to print a copy of this document for your records.
Step #3: Coaching Service Agreement
- This document will be emailed to you separately, so keep an eye on your inbox. It is set up to be easily completed online with a secure digital signature included. Simply follow the instructions given and hit submit when you are finished. A copy of the signed document will be emailed to both of us once it is complete. Please keep a copy of this agreement for your records.
Step #4: Complete the New Client Questionnaire
- This is the “get to know you” form that will help me get to know you and your needs better. Answers don’t need to be lengthy. To complete this form, click the button above and download the form to your own device or cloud storage account. Then simply type your answers in the boxes. Be sure to save your work as you go and when you are finished! You may submit the completed form either by emailing it back to me as an attachment or using the “Submit” button included on the form.
