Before we jump into all the good stuff that is waiting for you on this journey, there are a few housekeeping items we need to take care of…
Form Completion Instructions
Please download and review the Coaching Policies and Procedures document. This document contains everything you need to know about coaching, being coached, and a whole lot more…including office policies, confidentiality, and financial agreements. You will be asked to sign a statement that you have read and understand this document in the Coaching Service Agreement. You are advised to print a copy of this document for your records.
Returning or renewing clients
Please submit an updated Coaching Service Agreement prior to the end date of your current service contract in order to maintain continuous service. You will receive a copy of your submission via email after it is submitted.
Please complete and submit the New Client Intake Questionnaire and the Coaching Service Agreement at least 24 hours prior to our first session. If you are participating in a couple’s program, please have both partners complete a questionnaire. Only one person needs to do the service agreement, though. You will receive a copy of your submission via email after it is submitted.
